DESCRIPTION:Archdiocesan Applications available – Click here\nIf you attend a Catholic elementary school\, you will receive the Archdiocesan Application Form from your school principal. Your school principal will send us your school records\, transcripts\, and standardized test results. \nIf you do not attend a Catholic elementary school\, you can obtain an Archdiocesan Application Form from Dominican High School in the foyer office at the Walmsley Avenue entrance to the school or print one online here. You are responsible for obtaining the required records (grades 5 through your most current report cards and standardized test scores) from your school and forwarding them to us. Please see specific instructions on the application form. \nIf you attend a Catholic elementary school\, complete the application form you received and return it to your school principal. Your school principal will then forward your school records to Dominican. \nIf you do not attend a Catholic elementary school\, you are to return the application form\, report cards\, and test scores to Dominican according to the instructions on the application form.
