St. Mary’s Dominican High School will offer physicals through the Tulane Institute of Sports Medicine to athletes and members of the cheerleading and Deb squads. Any current and incoming students will have the opportunity to participate in these physicals if it works with their schedules. A student is not required to come on this day and could take the LHSAA medical form to her doctor or urgent care center.
The link to the physical examination/medical history form can be found on the athletics homepage of the Dominican website. Parents are asked to read and sign this form. Students will not be allowed to take a physical if this form has not been completed.
A student will not be allowed to tryout or participate on an athletic team unless there is a LHSAA physical on file at Dominican. This pertains to 8th, JV, and Varsity teams.

LOCATION: The Dance Studio located on the second floor of the Siena Center
COST: $20.00 – cash or check (check should be made payable to Dominican High School.)
